Dear Parents,
The Department of Health has provided the appointment dates and times for students enrolled in the School Dental Care Service. Your child was given an individual appointment slip to take home this week. If you have not received it, please contact your child’s class teacher or me as soon as possible.
Important: Green Appointment Slips (Payment Required)
If your child received a green appointment slip, it means payment for the dental programme has not yet been settled.
- Action Required: Please scan the QR code on the green slip and complete the payment before the clinic date.
- Please Note: Failure to settle the payment beforehand will result in the cancellation of your child's dental examination.
What to Bring on the Appointment Day
Please ensure your child brings the following items to the clinic:
- Identity Document: (e.g., HKID, HK Birth Certificate, or Passport with a valid HK Visa)
- Appointment Slip (The slip sent home this week)
- Toothbrush
- Student Handbook for the School Dental Care Service (if they have one)
