Dear Year 6 Parents and Guardians,

I would like to inform you that the School Immunisation Team (SIT) of the Department of Health will visit our school on Monday, 2 February 2026 to provide the following childhood immunisations:

1. Measles, Mumps, Rubella & Varicella (MMRV) or Measles, Mumps & Rubella (MMR) Immunisation
2. Human Papillomavirus Immunisation – female students only
3. Hepatitis B (HBV) – for those Year 6 students who have never received or have not completed three doses of HBV according to the immunisation schedule.

The consent form has been sent out today. Please complete the consent form or refusal form by Monday, 12 January 2026.

Please indicate the availability of the immunisation record as shown in the image below.

To complete the application process, after signing the consent form, you must provide your child’s original copy of all immunisation records to the nurse’s office or your class teacher before the deadline. If you consent to the vaccination, but do not provide all the documentation required, SIT will issue a referral letter to arrange another appointment for your child.

SIT will also check the immunisation records of the students to see their history of BCG vaccination. Students without a history of BCG vaccination will be referred to the Chest Clinic of the Department of Health for management.
