Dear Year 6 Parents and Guardians,

I would like to inform you that the School Immunisation Team (SIT) of the Department of Health will visit our school on Friday, 7 November 2025, to provide the following childhood immunisation:

• Combined Diphtheria (reduced dose), Tetanus, acellular Pertussis (reduced dose) & Inactivated Poliovirus Vaccine (dTaP-IPV)

The consent form has been sent out today, please complete the consent form or refusal form by Friday, 17 October 2025.

Please indicate the availability of the immunisation record.

To complete the application process, you must provide your child’s original copy of all immunisation records to the nurse’s office or your class teacher before the deadline. If you consent to the vaccination, but do not provide all the documentation required, SIT will issue a referral letter to arrange another appointment for your child.

SIT will also check the immunisation records of the students to see their history of (1) BCG vaccination, (2) Measles, Mumps & Rubella (MMR)/ Measles, Mumps, Rubelle & Varicella (MMRV) Vaccination and (3) Hepatitis B (HBV). Students without a history of BCG vaccination will be referred to the Chest Clinic of the Department of Health for management. For students who have not completed MMR or HBV vaccination, they will receive catch-up vaccination in the second term school visit.
