Dear Parents,

The Department of Health has now provided us with appointment dates and times for pupils who have enroled in the Student Health Services.

An appointment slip issued by the Student Health Services with details of your child’s appointment date and time was given to your child to take home this week. Please check with the class teacher or me if you do not have the slip.

Please ensure your child brings their Identity document (e.g., HKID, HK Birth Certificate, Passport with HK Visa), appointment slip and past medical records.
